Portfolio Project Directions and Rubrics
This Portfolio Project is worth 15% of your overall grade.
Completing this Assessment will help you to meet the following:
Course Outcome
• Abstract and apply the advanced CPT and HCPCS level II classification requirements in accordance with the official coding guidelines.

• Abstract information from medical charts to assign the correct diagnosis (ICD-10), procedure (CPT) and supply (HCPCS Level II) code in a variety of medical situations.

Scenario
You are a physician′s office Coding Educator for Premier Medical Group. As a Coding Educator, you are responsible for onboarding new Certified Coders. Certified Coders review patient records, identify what information they need to abstract from the documentation, and then assign the correct code(s) to that information. One of the important reasons for accurate assignment is to ensure that services rendered are reimbursed appropriately.
As a Coding Educator, you have been tasked with creating a coding training aid for new Certified Coders that will contain codes for a patient′s diagnosis, procedures completed, and drugs and supplies used. To create this training aid, you must review the provided patient′s records, assign appropriate codes, and provide rationale as to why the selections were made.
Directions
For this project, you will review a patient′s records, which will be provided in Blackboard over the next several weeks, and you will identify diagnosis codes, procedural codes, and drug and supply codes when applicable. You will record the identified codes in the Coding Template, which you will download in Blackboard. Each week, you will add additional codes to this template. The complete template will serve as your coding training aid for new Certified Coders.
Deadline
Deliverable items for the Portfolio Project will be required at different points during the course. Details for each submission are included below in the Deliverable Desсrіptions.

Week 2: Rodney Balbo′s Emergency Room Visit
• Due by the end of Week 2 at 11:59 pm, ET. – Due Saturday, 11/9/2024 @ 11:59 PM ET
1. Download the Emergency Room Visit Note in Blackboard, review it, and then assign the appropriate ICD-10-CM, CPT, and/or HCPCS Level II code(s) on the template provided.
2. On the same template, in 250 words or more, reflect on your method for finding the codes for this patient. Describe your steps to ensure you have this coded properly.
Week 3: Rodney Balbo′s Office Visit
• Due by the end of Week 3 at 11:59 pm, ET. – Due Saturday, 11/16/2024 @ 11:59 PM ET
1. Download the Office Visit Note in Blackboard, review it, and then assign the appropriate ICD-10-CM, CPT, and/or HCPCS Level II code(s) by adding on to the template.
2. On the same template, in 250 words or more, reflect on your method for finding the codes for this patient. Describe your steps to ensure you have this coded properly.
Week 4: Rodney Balbo′s Imaging Service
• Due by the end of Week 4 at 11:59 pm, ET. – Due Saturday, 11/23/2024 @ 11:59 PM ET
1. Download the Imaging Service Note in Blackboard, review it, and then assign the appropriate ICD-10-CM, CPT, and/or HCPCS Level II code(s) by adding on to the template.
2. Using Medicare′s website, determine whether or not the CPT has an active LCD and identify the LCD Article ID Number by adding on to the template.
3. On the same template, in 250 words or more, reflect on your method for finding the codes for this patient and include based on the information from the LCD Article, if the diagnosis codes meet medical necessity. Describe your steps to ensure you have this coded properly.
Week 5: Rodney Balbo′s Outpatient Surgery #1
• Due by the end of Week 5 at 11:59 pm, ET. – Due Saturday, 11/30/24 @ 11:59 PM ET
1. Download the Outpatient Surgery #1 Note in Blackboard, review it, and then assign the ICD-10-CM, CPT (for the surgery and anesthesia), and/or HCPCS Level II code(s) by adding on to the template.
2. Assign the appropriate APC by adding on to the template.
3. On the same template, in 250 words or more, reflect on your method for finding the codes for this patient. Describe your steps to ensure you have this coded properly.
Week 6: Rodney Balbo′s Outpatient Surgery #2
• Due by the end of Week 6 at 11:59 pm, ET. – Due Saturday, 12/7/2024 @ 11:59 PM ET
1. Download the Outpatient Surgery #2 Note in Blackboard, review it, and then assign the appropriate ICD-10-CM, CPT (for the surgery and anesthesia), and/or HCPCS Level II code(s) by adding on to the template.
2. Assign the appropriate APC by adding on to the template.
3. On the same template, in 250 words or more, reflect on your method for finding the codes for this patient. Describe your steps to ensure you have this coded properly.
Week 7: Final Submission to ePortfolio
• Due by the end of Week 7 at 11:59 pm, ET. – Due Saturday, 12/14/2024 @ 11:59 PM ET
This is the final submission of your Portfolio Project. Incorporate instructor feedback, make any additional corrections, and submit to your ePortfolio.
